Case management

The HIV Care Coordination Program makes sure people with complex needs get the right services at the right time, without having to assemble the system themselves. You participate in every part of the process. Nothing is decided about your care without you.

What a case manager does

The main job is arranging services across different providers and settings: assessment, intervention, evaluation, and advocating on your behalf when a service you need is hard to get. If you are newly diagnosed, this is the person who helps you figure out what happens next.

Ending the HIV Epidemic

Ending the HIV Epidemic is a federal initiative from the U.S. Department of Health and Human Services. Butler County Health Department runs the prevention side across 20 counties in the Southeast Region. A navigator travels the region to raise awareness and connect people to testing, prevention, and care.
