What the program does
There are two parts. Service coordination is available to every participant no matter what their finances look like. Funding help is limited and is available only to participants who are medically eligible and meet the financial guidelines.
Service coordination
Outreach, referral, eligibility determination, needs assessment, and coordinating care across providers.
Limited funding
Payer of last resort for eligible services.
Payer of last resort
The Missouri Department of Health and Senior Services pays only after other sources have been used. Your service coordinator will help you find and apply for other coverage first, including insurance and MO HealthNet. That is part of the job, not an obstacle to it.
Talk to a coordinator
Call (573) 785-1013 or 1-800-747-6818. You can call before you know whether your child qualifies. The coordinator will walk you through it.
